Homecare
Criteria

Homecare admission criteria are as follows:

• Patient is homebound as required by payor source.


• Patient is under the care of a licensed physician.


• Patient has a specific need for care as defined by the appropriate federal or state program.


• Patient lives in Sumner, Robertson, Davidson, Wilson, Smith, Macon or Trousdale county.


• Sumner Homecare is able to provide the care needed.


• The home environment is suitable for providing care.


• Acceptance for services is based on the patient’s and caregiver’s ability to function in the place of residence.

Discuss home care options with your physician, who will make a referral to the agency of your choice. A social worker or discharge planner in a health care facility may also assist with the process. Hospice is available with a physician’s order to patients with:

• Medicare/HealthSpring


• Private insurance


• Workers’ compensation


• Private pay
