Discharge Planning

Discharge planning is a process that begins upon patient admission to TMC that creates a personally customized plan to meet each individual need of the patient upon discharge from our facility. Discharge planning includes plans for accessing community and governmental services, such as social agencies, that may be able to complete the care continuum for patients after leaving the hospital.
